RMA Request Form
Prior to returning any unopened merchandise, we strongly advise that you carefully review our return policy guidelines. If your purchase meets our return eligibility requirements, you may complete the RMA form below to request an RMA#. Absolutely no returns shall be accepted by Jellen Skin Care Products unless accompanied by an original or emailed sales invoice and a Returned Merchandise Authorization Number (RMA#). Any invalid or unauthorized returns will be refused by our warehouse and returned to sender at sender's expense. |
